Repatriation of Deceased Remains

REQUIREMENTS FOR SHIPPING REMAINS OF A ZIMBABWEAN NATIONAL TO ZIMBABWE:

1. Copy of Death Certificate
2. Copy of Post-Mortem Report
3. Copy of Embalming Certificate
4. Copy of Non-Infectious Diseases Certificate
5. Copy of Permit to repatriate remains
6. Copy of Zimbabwean passport (pp1-5 for old type passport, last page for new type passport)
7. Non-Hazardous/ flammable goods declaration affidavit (by Funeral Home)
8. Service fee payable to Embassy of Zimbabwe
9. Original documents cited above to be sent with the remains.
10. An affidavit/letter from a representative of the deceased’s family requesting that the body be buried in Zimbabwe is required.

N.B: No body shall be repatriated to Zimbabwe without clearance from the Embassy.In case where the deceased does not have Zimbabwean document(s), the Embassy must be provided with the following additional documents:

1. A Zimbabwean national ID card and / or Zimbabwean birth certificate of the informant
2.  Affidavit written by the next of kin stating that the deceased person is indeed a     Zimbabwean National and stating the nature of the relationship with next of kin.
3.  The Embassy will then seek the necessary clearances from Harare.

THE REPATRIATION PROCESS

Where all the required documents are available, the undertaker should approach funeral directors in Zimbabwe directly to avoid unnecessary delays in processing repatriation of human remains to Zimbabwe. The Embassy should be approached only in those cases where the deceased’s passport cannot be found.

N.B. The above stated documents should reflect the name on Zimbabwean identity documents of the deceased. If the human remains are transported to Zimbabwe by air, all documents mentioned above should be faxed directly to the undertakers in Zimbabwe together with contact details (telephone numbers and names) of the undertakers in Sweden, Nordic or Baltic countries. Sending to Zimbabwe inadequate documents will automatically delay the process.

Deceased without a passport

Please note that in cases whereby the deceased does not have a passport, the undertakers should approach the Embassy with the following additional documents:

i) Zimbabwean national identity card and/or Zimbabwean birth certificate;

ii) Affidavit written by the next of kin stating whether the deceased had a passport or whether it was lost and whether he/she never had a passport. If the relatives do not know anything pertaining to the deceased ever having a passport, they should state so in the affidavit.
